Mechanical function of the left atrium is improved with epicardial ligation of the left atrial appendage: Insights from the LAFIT-LARIAT Registry

Abstract

Left atrial (LA) strain (Ƹ) and Ƹ rate (SR) analysis by two-dimensional speckle tracking echocardiography (STE) is a novel way of functional assessment of LA. We studied the impact of LA appendage (LAA) exclusion by LARIAT epicardial ligation on the mechanical function of LA by performing Ƹ and SR analysis before and after the procedure. A total of 66 patients who underwent successful LAA exclusion were included in this study. Of 66, thirty-two patients had adequate paired data for Ƹ and SR analysis. SR during ventricular systole (LA-SRs) represents LA reservoir function and SR during early ventricular diastole (LA-SRe) represents LA conduit function. Ƹ and SR were determined from apical four and two-chamber views using electrocardiographic QRS as reference point. LA volume index (LAVI), as surrogate for LA remodeling, was measured from apical views. The mean age of the patients was 70 (± 9.2) years. LAA ligation resulted in improved reservoir function [LA-SRs: 0.72; confidence interval (CI): 0.63 to 0.83 versus 0.81; CI: 0.73 to 0.98); p value = 0.043] and conduit function (LA-SRe: 0.74; CI: 0.67 to 0.99 versus 0.89; CI: 0.82 to 1.07; p value=0.025]. LAVI improved significantly with LARIAT (35.4; CI: 29.4 to 37. 2 versus 29.2; CI: 28.2 to 35.9 p value < 0.023). LAA exclusion appears to improve mechanical function of LA and also results in reverse LA remodeling.
