@wendyburn Discharge communication is itself a complex systems problem- simplifying it too much can be a trap. As the patient becomes more complex, what GPs do and don’t want seems to be decreasingly standardisable - level of detail etc. Very tricky! 🧵 htt

@BenAllenGP It’s the long term tribal backdrop that can weaponise a heading like ‘actions for gp’. Next step for discharge summaries is getting 2ary care folks to really understand 1ary care perspectives by sharing feedback. Reduces the tribal side too. ht
